We are looking for someone who values and wishes to work with older people. Previous experience is helpful but if you have the passion to support our clients, transferable skills, common sense, an organised and flexible approach to work and a ‘can do’ approach, we are looking for you.
The Home from Hospital service provides older, vulnerable people with low level support for a limited period after hospital discharge. The purpose of the service is to ensure people have a successful discharge by accessing help and support and regaining their confidence to enable them to return to independent living.
As a Co-ordinator you will be responsible for carrying out client assessments, developing personal support plans with the client, matching clients with volunteers who help our clients achieve their goals, supervising volunteers and monitoring progress to ensure our clients achieve a successful outcome. This role also supports the senior coordinator in promoting the service as well as recruiting and supporting volunteers.
The successful candidate needs to have a full driving licence and use of a car.
