We are looking to appoint 2 new Engagement Workers who will provide support to people experiencing housing vulnerability to access and engage with health care providers. The project works with people upon discharge from hospital, or referred by GP surgeries or the voluntary sector, to ensure that their health recovery is sustained and that they receive relevant assistance with their broader needs, linking in with the wide range of services which are available in the city.

The Creative Change project, part of Justlife, supports people vulnerably housed in Brighton & Hove. We have an exciting opportunity to appoint a new Project Worker who will provide support to people in emergency and unsupported temporary accommodation. The work will involve taking on a caseload of homeless and vulnerably housed people and supporting them through a number of interventions.
